Patient safety is a fundamental priority in healthcare services and is strongly influenced by the effectiveness of incident reporting systems and nurses’ responsiveness in reporting and managing patient safety incidents. The Online Incident Reporting System (OIRS) was developed to improve systematic and sustainable incident reporting practices. This study aimed to analyze differences in nurses’ responsiveness and patient safety levels before and after the intervention, to examine the effect of changes in OIRS implementation on nurses’ responsiveness and patient safety, and to analyze the mediating role of nurses’ responsiveness in the relationship between OIRS implementation and patient safety. This study employed a quasi-experimental design with intervention and control groups. The sample consisted of nurses at RSKB Columbia Asia Pulomas selected based on inclusion criteria. The intervention involved the implementation of the Online Incident Reporting System (OIRS). Data were collected using structured questionnaires and analyzed using the Wilcoxon Signed Rank Test, Spearman correlation, and mediation analysis. The results demonstrated significant differences in nurses’ responsiveness and patient safety levels before and after the intervention. Changes in OIRS implementation had a significant effect on improving nurses’ responsiveness and patient safety. Furthermore, nurses’ responsiveness was found to act as a mediating variable in the effect of OIRS implementation on patient safety outcomes. This study concludes that the implementation of the Online Incident Reporting System (OIRS) is an effective strategy for enhancing patient safety through strengthening nurses’ responsiveness. The integration of technology-based incident reporting systems with organizational support is essential to ensure sustained improvements in the quality and safety of nursing care.
