HUBUNGAN KADAR LAKTAT DAN JUMLAH EOSINOFIL DENGAN MORTALITAS PASIEN SEPSIS DI ICU RSUD WALED KABUPATEN CIREBON 2020-2024

Abstrak

Latar Belakang: Sepsis adalah disfungsi organ yang mengancam jiwa akibar respons tubuh yang tidak teratur terhadap infeksi dan merupakan penyebab utama mortalitas di ruang perawatan intensif (ICU). Kadar laktat dan jumlah eosinofil merupakan biomarker potensial untuk prognosis pasien sepsis. Peningkatan kadar laktat mencerminkan hipoksia jaringan, sedangkan eosinopenia berhubungan dengan respons inflamasi sistemik. Tujuan: Mengetahui hubungan kadar laktat dan jumlah eosinofil dengan mortalitasi pasien sepsis di ICU RSUD Waled Kabupaten Cirebon Tahun 2020-2024. Metode: Penelitian ini menggunakan desain observasional analitik dengan pendekatan cross-sectional. Data sekunder diambil dari rekam medis 38 pasien sepsis yang dirawat di ICU dan memenuhi kriteria inklusi. Analisis data dilakukan dengan uji Mann Whitney. Hasil: sebagian besar pasien mengalami hiperlaktatemia (71,1%) dan eosinopenia (84,2%). Mortalitas tercatat sebesar 84,2%. Terdapat hubungan bermakna antara kadar laktat dan mortalitas (p = 0,000) serta antara jumlah eosinofil dan mortalitas (p = 0,000). Simpulan: kadar laktat yang tinggi dan jumlah eosinofil yang rendah ebrhubungan dengan peningkatan mortalitas pasien sepsis di ICU, sehingga dapat digunakan sebagai indikator prognostik awal.

Background: Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection and remains a leading cause of mortality in intensive care units (ICUs). Lactate levels and eosinophil counts are potential biomarkers for the prognosis of sepsis patients. Elevated lactate levels reflect tissue hypoxia, while eosinopenia is associated with a systemic inflammatory response. Aim: To determine the relationship between lactate levels and eosinophil counts with mortality in sepsis patients in the ICU of RSUD Waled, Cirebon Regency, during 2020–2024. Methods: This study used an analytical observational design with a cross-sectional approach. Secondary data were obtained from the medical records of 38 sepsis patients treated in the ICU who met the inclusion criteria. Data were analyzed using the Mann-Whitney test. Results: Most patients experienced hyperlactatemia (71.1%) and eosinopenia (84.2%). The recorded mortality rate was 84.2%. There was a significant correlation between lactate levels and mortality (p = 0.000) as well as between eosinophil counts and mortality (p = 0.000) Conclusion: elevated lactate levels and low eosinophil counts are associated with increased mortality in sepsis patients in the ICU and therefore can be used as early prognostic indicator.
